Implementation of Fracture Liaison Service in the Czech Republic

The main goal of the rigorous work was to verify the feasibility of implementing a coordinated care program for patients after osteoporotic fractures (so-called low traumatic fractures) of the so-called Fracture Liaison Services (FLS) system in the Czech Republic from the point of view of nurses' access. Sub-objectives of the quantitative research mapped the current status of adherence to procedures in the care of patients after low-traumatic fracture from the perspective of nurses, attitudes of nurses towards implementing FLS, and access to continuing education for the position of FLS coordinator. The sub-objectives of the qualitative research compared the development of care for osteoporotic patients in the Czech Republic between 2010 and 2019. They compared the situation in Lithuania, Romania, and Slovakia between 2010 and 2019.
Subjective perception of workload and fatigue of nurses

The bachelor's thesis entitled Subjective perception of workload and fatigue of nurses deals with the issue and perception of the degree of workload, stress and fatigue of nurses. The aim of the thesis is to determine the subjective perceptions of workload of selected groups of nurses and then to evaluate the results of the research and suggest recommendations for practice. The theoretical part of the thesis contains a basic overview of the individual knowledge on the subject, describes the types of workload and fatigue and points out its consequences and prevention. The empirical part contains the results of our own research using a standardized Meister questionnaire. Furthermore, the results of the own investigation were compared with the results of other similar research on a similar topic. The method of the survey was a self-constructed survey, which was supplemented with the Meister questionnaire. The questionnaire was distributed to nurses employed at Motol Hospital in intensive care beds. The sample consisted of 94 respondents. Results: Considering the perceived burden, it was found that 44 % of the respondents felt that they did not have sufficient knowledge of relaxation techniques and activities to reduce the burden. Experience with volunteers from the perspective of nurses

Aim: The aim of the bachelor's thesis is to map and evaluate the experience of nurses with volunteers who come to selected inpatient wards at the Motol University Hospital, and to assess whether the experience of nurses with volunteers is rather positive or negative. The work is divided into a theoretical and an empirical part. The theoretical part contains a basic definition of the concepts of volunteering, in healthcare facilities in the Czech Republic and abroad. The empirical part deals with the research investigation itself. Methods: The research was carried out in the form of a non-standardized questionnaire, which contained twenty-three questions. This questionnaire was distributed to selected respondents, specifically to nurses in selected departments of the University Hospital in Motol. Data collection took place from December 2022 until the beginning of March 2023. Results: A total of fifty-seven fully completed questionnaires were obtained from the nurses. In the questionnaire, 82.46 % of the nurses evaluated the involvement of volunteers in care positively, 15.79 % rather agreed with the positive experience. Post-qualification education of urological nurses in Czechia and other EU countries

This bachelor's thesis is focused on mapping out the possibilities of post-qualification education of nurses working in the field of urology in the Czech Republic and in chosen countries of the European union, which are Sweden, Ireland, Denmark, Netherlands, Germany, and Poland. It documents the history of education of nurses, as the history of urology. It offers a more detailed description of the current educational system of nurses and the possibilities of post-qualification specialisation in the Czech Republic and the six chosen countries. In the discussion part of the thesis is a summary of information from scientific studies and sources relevant to the topic pointing to the benefits of wider possibilities of post- qualification education of nurses in urology, and to the importance of a continuing education.
Nursing Perioperative Care in the Cardiac Surgery Department

The incidence of civilisation diseases is rising not only in the Czech Republic but also worldwide. The incidence of cardiovascular diseases is widely represented in the population. To ensure proper and continuous development in the field of cardiac surgery, seamless interdisciplinary cooperation is necessary, which includes also perioperative nurses. The aim of this thesis is to identify the specifics of nursing care provided by perioperative nurses in the cardiac surgery department and to determine whether these nurses are motivated in the area of lifelong education. The empirical part consisted of both qualitative and quantitative research. Qualitative research was conducted through semi-structured interviews with perioperative nurses working in cardiac surgery operating theatres. Nurses were deliberately selected from three different healthcare facilities. The interviews were transcribed verbatim, processed using the "pen and paper" method, and categorized. For the quantitative research was used a non-standardized questionnaire which consisted of 18 questions and was distributed among perioperative nurses working in cardiac surgery operation theatres throughout the Czech Republic. Distribution was also carried out through NCO NZO (National Centre of Nursing and Non-medical Healthcare fields) and the Faculty of Health Studies in Pardubice. A total of 62 respondents participated in the questionnaire survey. The resulting data was processed using MS Excel and the Realstatistics add-on, with the help of graphs, contingency tables, and statistical calculations. The results of the qualitative research allowed us to map the activities performed by perioperative nurses in the role of a scrub or circulating nurse in cardiac surgery operating theatres. Perioperative nurses carry out a wide range of tasks which include care before, during and after the surgery. Communication and proper functioning of the entire surgical team are also essential. By carefully preparing for the procedure, complying with all duties and fulfilling good workplace relationships, they can provide the best possible care to patients. The results of the quantitative part of the research showed that there is no correlation between years of experience and the motivation of perioperative nurses in cardiac surgery departments for lifelong education, or between the highest level of education attained and their motivation for lifelong education. Despite this, the research had positive results. We found that all perioperative nurses in our research sample are sufficiently motivated for lifelong education. This thesis and its results can be valuable by newly employed perioperative nurses. They can learn basic information about the work of perioperative nurses in the cardiac surgery department and about opportunities for specialization or lifelong education. The section of lifelong education may also be supportive for all other nursing fields.
Hostile and aggressive behaviour of patients towards nurses in clinical practice

Current state: Aggressive and hostile behaviour of patients towards nurses in clinical practise is an alarming and common phenomenon that jeopardizes the safety and well-being of healthcare personnel. Objectives: The first objective of this thesis was to evaluate the number of adverse events in the "Behaviour of Individuals" category reported to the "National Portal for Reporting Adverse Events" during the period 2018-2021. The second objective was to assess the occurrence of aggressive behaviour of patients toward nurses in clinical practise. The third objective was to describe a spectrum of preventive nursing procedures that minimize the risk of aggressive behaviour of patients towards nurses. Methodology: The first research method involved a secondary analysis of statistical data from the National Portal for Reporting Adverse Events (ÚZIS ČR, 2022). The second research method utilized a quantitative study conducted through the administration of a standardized questionnaire called "Aggression of Patients towards Nurses in the Course of Their Profession (Lepiešová et al., 2015) ". This survey technique involved nurses from internal and surgical departments. Additionally, third research method was used, and it involved qualitative research through in-depth interviews with nursing care managers. Results: The results indicated that the most frequently reported event to the National portal of Reporting Adverse Events was category called "pressure ulcers ". The results of the qualitative study showed that patient aggression was not related to the field of care (p= 0.587). Furthermore, the results revealed that nurses with longer clinical experience encountered aggression from patients more frequently (p< 0,001). The results of the qualitative study described the most common preventive measures to minimize the risk of patient aggression towards nurses, including training and education of nurses and proper reporting of adverse events, addressing staff shortages, use of restraining measures and pharmacological interventions and appropriate communication with patients and their families. The hospital management prepares for the risk of patient aggression through staff training. Conclusion: The practical outcome of this diploma thesis is a set of recommendations for non-medical healthcare personnel.
Taking Care of Prostatectomy Patients Making Use of the Callisty Roy Nursing Programme

This diploma thesis deals with nursing care for prostatectomy patients using Callista Roy´s nursing programme. The situation, after prostate surgery, is a new life experience for men, that takes adaptation. Callista Roy, the author of Adaptation Model, was dealing with patient adaptation. The diploma thesis has set three aims. The first aim was to find out what interventions are used by nurses to support patient adaptation after prostatectomy in individual Callista Roy´s adaptation models. The second aim was to find out what stimuli nurses observe while providing prostatetomy patients with nursing care, and the third aim was to find out what problems prostatectomy patients face in individual adaptation modes. Three research questions were determined for these three aims. The diploma thesis contains two research files. The first research file are Urological Intensive Care Unit nurses and the other one are patients hospitalized at the Urological Standard Unit. Data was collected using qualitative research, methodology of questionnaires, interviewing technnique in prostatectomy patients at a hospital facility and nurses taking care of these patients. The survey is made complete by participant observation. The survey was carried out in February and March 2023. Nurse interview results have proven that nurses apply elements of the adaptation physiological mode when providing prostatectomy patients with nursing care. Patient interview results have proven they do not observe any problem in adapting to hospital environment nor in adapting to the state after having their prostate removed. The patient interview results have implicitly proven that they downplay the problems, and they would appreciate some help supporting their adaptation. The results of this diploma thesis may serve as a study material for healthcare field students. The diploma thesis may serve as inspiration for nurses supporting patient adaptation to carried-out prostatectomy.
Nursing physical examination in surgical nursing care.

Current Status: The physical examination is an essential component of patient assessment, contributing to accurate diagnosis. It encompasses techniques such as inspection, palpation, percussion, auscultation, and digital rectal examination. Thesis Objectives: This study aims to delineate nursing actions in conducting physical examinations and highlight disparities in performing these assessments within surgical nursing settings. Methodology: A qualitative research method was employed, utilizing semi-structured interviews. The interview cohort comprised 8 nurses working in various surgical wards. Furthermore, a quantitative survey, comprising open, semi-open, or closed questions, was selected and included 169 respondents practicing across diverse surgical units. Data collection was conducted online, and participant selection was guided by data saturation. Thesis Findings: Interviews underscored nurses' adeptness in conducting physical examinations and their awareness of nuances that vary across departments. Most respondents (42.6%) hailed from traditional surgical units. A significant portion held a bachelor's degree (45.6%), and the prevalent experience range within surgical wards was 0-10 years (56.8%). Notably, 73.4% of patient admissions involved collaborative data collection between nurses and physicians. During physical examinations, 85.2% of nurses helped, and 97.6% performed assessments within their respective units. Inspection, olfaction, and assessment of consciousness consistently fell under nursing purview. Palpation and auscultation were infrequently conducted by nurses, with digital rectal examinations being exceedingly rare. Physical examination by a nurse varies from one surgical department to another. A relationship between the palpation and consciousness examination and the surgical ward in which she works has been demonstrated. Physical examination by a nurse varies with length of experience. Conclusion: In summary, nursing-conducted physical examinations is performed by the nurse within the scope of her competence and align with departmental needs and resources, reflecting the specific care context. The findings emphasize the collaborative nature of patient assessment and shed light on areas where nursing participation may be enhanced.
The issue of providing nursing care in acute psychiatric wards in connection with the covid-19 infection

This bachelor thesis focuses on the providing of nursing care in acute psychiatric wards in the context of Covid-19 infection. The theoretical part of the thesis starts with an overview of the organization of psychiatric care in the Czech Republic. It then focuses on selected acute conditions of patients admitted to acute psychiatric wards, including schizophrenia, neurotic, stress and somatoform disorders, and mental health disorders caused by alcohol and psychoactive substance use. Special attention is given to Covid-19 disease and its impact on acute psychiatric wards. The theory also addresses the providing of nursing care during the Covid-19 pandemic, including hygiene measures. The empirical part of the thesis is developed through a qualitative investigation with a semi-structured interview technique. Using open coding, the data was divided into three categories and six sub-categories. The research population, consisted of 10 informants. They were nurses, from different facilities, who work in acute psychiatric wards and who worked in this ward during the Covid-19 pandemic. Their experience in health care ranges from 4 to 49 years. The sample was selected by purposive sampling. The final part of the thesis focuses on the impact of the Covid-19 pandemic on acute psychiatric wards and nursing care. The results of the data analysis provide valuable insights into this issue. One of the key areas examined was changes in the operation of psychiatric wards due to the pandemic. Some wards were reconfigured into specialist covide wards, while others were divided into sections for patients with and without the disease. This measure involved intensive training of staff to work in an infectious environment and increased monitoring of patients for possible infection.
Nurse education about thromboembolic disease about their risks and prevention.

The bachelor thesis titled Nurse education about thromboembolic disease about their risks and prevention deals with patients' knowledge regarding the thromboembolic disease, education activities of nurses and determining the effectiveness of such education activities of nurses regarding thromboembolic disease. The theoretical part focuses on the anatomy of the vascular system and the heart as well as on the process of blood clotting. Furthermore, the theoretical part is devoted to the characteristics of thromboembolic disease, its symptoms and risk factors. The diagnosis, treatment and prevention of thromboembolic disease are also presented. Finally, the nursing care of patients with phlebothrombosis and pulmonary embolism and patient education are described. Three objectives were set for this bachelor thesis. The first objective was to map the patients' knowledge of thromboembolic disease, about its risks and prevention. The second objective aimed to map the educational activities of nurses in the context of thromboembolic disease. The third objective was to discover how nurses determine the effectiveness of educational activities about thromboembolic disease. Based on these, following three research questions were established: What do the patients know about thromboembolic disease, about its risks and prevention? What kind of educational activities do the nurses in the hospital do regarding the prevention of thromboembolic disease? How do nurses determine the effectiveness of educational activities about thromboembolic disease in the hospital? In order to obtain the results of the research investigation, two research sets were established. The first research set consisted of the nurses providing care and the second research set consisted of the patients. These patients were hospitalized in the ward where the nurses worked. The actual research investigation was conducted by meeting the nurses and patients in person using qualitative method and semi-structured interviews. All interviews were recorded with the consent of the persons on a mobile device voice recorder. The data collected from the interviews were processed and analyzed using the pencil-and-paper method. The main categories and sub-categories were created based on these data. Three and seven subcategories were established for nurses providing care. There were three categories and six established for the patients. The first research question asked was: What do the patients know about thromboembolic disease, about its risks and prevention? According to the research conducted, it was found that patients do not have enough information about thromboembolic disease and are not fully informed by nurses in the hospital about the symptoms of phlebothrombosis and pulmonary embolism, risk factors and preventive measures of thromboembolic disease. Within the second research question, it was asked: What kind of educational activities do the nurses in the hospital do regarding the prevention of thromboembolic disease? The research showed that nurses educate patients about the importance of exercise, any activity, the proper application of low molecular weight heparin before discharge to home treatment and the proper loading of compression bandages before surgery. Regarding the third research question, How do nurses determine the effectiveness of educational activities about thromboembolic disease in the hospital?, the investigation showed that the most important thing for nurses is communication with the physiotherapist, monitoring the patient at each room visit as well as obtaining the feedback from the patient after education on the correct application of low molecular weight heparin. Based on the results of the survey, an information leaflet on thromboembolic disease has been developed and will be provided to private practices or hospitals.
